Liability and Photo Release: I acknowledge that volunteering for LifeMoves may involve a risk of harm or injury, including bodily injury. In consideration of being permitted to participate in LifeMoves volunteer activities, I agree to assume all risks involved in these activities, whether or not the risks are apparent to me. On behalf of myself, my heirs, and legal representatives, and to the fullest extent permitted by law, I hereby release and discharge LifeMoves, its officers, directors, employees, agents and representatives, from and against any and all liability for injury, death or damages and/or any other claims, demands, losses or damages, incurred by me in connection with any aspect of volunteering for LifeMoves. I further grant full permission for LifeMoves to use any photographs, video or other recordings of me taken during my volunteering time for any legitimate and charitable purpose without monetary payment to me.

I have read the Photo Release and Release of Liability in its entirety and fully understand the contents thereof. I voluntarily agree to all terms, conditions, and all content of the Release.
